Welcome to the Kuemper Catholic Knights Online Registration!

Please read and follow these steps carefully to be fully registered.

All Athletes must have the following forms on file with the Kuemper Athletic Office before they will be allowed to participate.

Step 1: Athletes must have a current student participation form, health form, physical, insurance card copy and concussion waiver on file. 

Click on View My Account at the top right.

These forms are good for the current school year.

Step 2: On the last page before you hit submit you will fill out a username and password. 

You will use this username and password when you need to go into the registration to update your information.

Please keep your username and password in a safe place as we did not have a record of them. 

(YOU WILL HIT THE FAMILY LOGIN AT THE TOP OF THE PAGE TO GET BACK INTO YOUR FORMS 

ONCE YOU HAVE INITIALLY REGISTERED)

Athletes must have a current sport qualifying physical exam on file.  Physicals are good for 13 months from the exactdate of last physical.

Once you have your physical completed by a doctor, make sure parents sign the bottom on the doctor's side. 

The Physical and the insurance card copy can be downloaded when you are filling out the Activities/Sports Registration.

You may scan and upload the completed Physical Form when you register for a sport  (Step 4 - Physical Forms).

**If you miss to upload the Physical Forms during registration, you may login back to your family account later and upload the documents. 

Follow these steps:

1. Login to your family account.

2. Find the recently submitted registration and click on "View" under "Reg Form" column. 

3. You may directly click on Step 4-Physical Forms on the left side of the submitted form.

4. Upload the scanned Physical Forms in File Upload-Page 1 (Front) and File Upload-Page 2 (Back).

5. Click on "Save" at the bottom of the page after uploading the files.
